This was fine because humour is better when it is implied.
The image leaves one in no doubt about what is about to happen.

The section is about correct use of work chairs.
The caption reads: "Did you call, Roger? .............. Roger?"
The dropped file and the castor-tracks on the floor show clearly what Roger was doing
and the broken window shows clearly what happened.
It is easy to see what steps could have been taken to avoid this.

The section deals with the care one needs to take when visiting client premises.
A member of staff is pointing out that our hapless victim´s shoe laces are undone.
Had he payed attention to what was happening around him (an official making notes and an employee
clearly concerned with something happening above them),
he may have avoided the incident that is portrayed on the cover.
